A Case Report of the Median Cleft of Lower Lip

Abstract
The median cleft of lower lip is a very rare. The reported case in literature is only 6 cases up to 1950. Usually the median cleft of lower lip has many associated deformity which are tongue-tie, mandibular deformity and absence of hyoid bone.
In this case, the failure of mandibular union at symphysis and severe tongue-tie with alveolar defect is noted.
The cause of this rare congenital anomaly is still unknown but it seems to be same as. usual cleft lip and palate.
Embryologically main reason of this deformity may be the displacement of mesodermal element into the lower frenulum area.
That is why the tongue-tie is present in the median cleft lower lip. The surgical corrections was carried out as follows.
1) Removed fibrous band of frenulum and released tongue-tip with one large Z-plasty was done in the under surface of tongue.
2) The lip was repaired in usual fashion such as refreshing the cleft edge and sutured straight line on vermillion and lower lip but there is a thru and thru Z plasty at base of cleft.
3) The floating mandible is left alone until body became old enough.
